Who Qualifies?

W-2 Employee

Participants must be W-2 employees.

Minimum 30 Hours

Employees must work a minimum of 30 hours per week to qualify.

Qualified Health Insurance

Must have qualified health insurance — through an employer, spouse, or private plan.
